Have an OHSAA Sports Physical

Must be VALID at the time of tryouts

Your physical should be valid for the duration of the sports season you are looking to participate in. If it expires before the season concludes, you are expected to submit an updated physical to be in compliance

 If physicals lapse, you will not be permitted to participate in practices/games/meets until a valid physical is submitted

Physicals can be uploaded directly into the Final Forms (preferred method), emailed to walnutatc@gmail.com, faxed to 513-363-8447 or submitted in person to the main office, athletic department offices or athletic training offices. Please avoid handing directly to coaches when possible

There are no exceptions to this expectation

Final Forms

This one-stop-shop streamlines collecting, updating and tracking of documentation (physicals, waivers, department forms) while protecting student data and complying with privacy regulations.

All families will have an account and be responsible for the completion of:

o   Register for sports of interest

o   Submitting parent forms

o   Submitting student forms

o   Uploading of physical

All forms must be submitted (by both families and student) by the first contest in their respective sport.

Please note that incoming families (new to Walnut Hills) will not have accounts until the school year rollover occurs – on/around July 1- so you will not be able to take care of this portion right away.

Program Directors are the first point of contact for program specific inquiries. If you have any sport specific questions (training dates, tryouts, etc.) please reach out directly to the program director who will be able to answer questions/connect you with the appropriate person within their program.

Athletics Participation Fee

The athletic participation fee allows us to maintain the scope of programs we provide and offer students a wide variety of opportunities in activities. The participation fee per sport is $70 (HS)/$40 (JH).

This fee helps to cover the costs of transportation, supplies, tournament fees and a number of other expenses that are no longer covered in the school district budget.

The payment will be paid online through Final Forms once the student has been selected for a team

The registration and fee should be completed by the end of the first week after tryouts or before the first contest. A student athlete will not be eligible to compete in athletic contests until the fee has been received.

IMPORTANT: Please be aware that the fee does NOT guarantee playing time, only the opportunity to be on the team if selected.

Families with concerns over the ability to pay fees should contact the Athletic Director.

Sports Schedules

Sports schedules are created based on space and team availability, within the windows of competition as outlined by OHSAA.

There are games often on weekends or over holiday breaks. Please review game and practice commitments prior to tryouts.Vacations during a student-athlete’s season are strongly discouraged. Vacations are not excused absences. Student-Athletes are expected to attend all practices and contests from the beginning of mandatory tryouts/practices until the sport season concludes. This includes fall break, winter break and spring break, depending on the season in which a student-athletes is participating.

Where can I get a Sports Physical?
Many local Urgent Care centers, Kroger Clinics, Walgreens, and CVS offer same-day physicals (no appointment needed). You may also use your primary care physician, but appointments can sometimes take longer—plan ahead. Be sure to bring your OHSAA sports physical form with you.

Should I keep a copy of my physical?

Always make and keep your own copy of your Sports Physical –this is why the parent upload into FinalForms is the preferred method. If we do not receive it from your child, or it is misplaced, you will need to provide another copy. A valid Sports Physical must be on file before your child can try out or practice. If it cannot be located, you may need to redo the exam at your own expense.

 

How do I know if my physical is still valid?

They are valid for 13 months following the completion by the physician. Verify via FinalForms or email paquetj@cpsboe.k12.oh.us for more information! 

 

When are tryouts?

While Athletics will try to update the website with the most current tryout information, things are constantly changing. Please contact the Program Director for the most current information.


What if I don’t make the team?

Athletics Participation Fees should not be done until after team selection. Students who quit after the deadline for the payment of fees (the first contest for their respective sport) will NOT have the fee refunded.

 

Where can I find game schedules?

Games for the upcoming school year will be available no later than July 1. Each team has their respective page on the website, game schedules for each level can be found there. Additionally, game schedules can be found here: Walnut Hills Schedule (select the sport in question to see the schedule)
